How to apply

The Faculty of Pharmacy and Pharmaceutical Sciences offers two intakes per year for the Graduate Certificate in Pharmacy Practice.  Semesters commence in March and July. 
   
Applications for semester 1, 2010 close on Monday 30 November, 2009

Application process

Graduate Certificate in Pharmacy Practice registered pharmacist stream Course Code 3460 –complete:

• the online e-admissions application and upload your:
  • curriculum vitae (CV)
  • academic transcripts*
  • proof of citizenship status*
  • proof of registration.
• Pharmacy Practice Application Supplementary Form Reg Pharm Stream
  • You will require your online applicant email address or your Monash student number to complete this form
  • You are required to indicate the subjects/units you wish to enrol into for Semester 1 and 2

Please include on your CV all pharmacy related work experience, additional other work experience, pharmacy related committee involvement and any other committee and community involvement.  Please do not include any student placements that are a component of the Bachelor of Pharmacy/Master of Pharmacy.

Monash pharmacy graduates do not need need to provide their academic transcripts.
