Application and enrolment information

You may elect to enrol in either:

The Graduate Certificate in Pharmacy Practice (Course code 3460 ) (24 points) comprising:

• VCP5011 Applied pharmacy practice I (6 points)
• VCP5012 Applied pharmacy practice II (6 points)
• Elective units totalling 12 points

OR

Single unit enrolment (Internship program only) (Course code 3461 )  (12 points) comprising:

Semester 1, 2010  intake applications close Monday 30 November  2009. 

To apply all applicants must complete both these online forms:

1. Submit your course application via the online application portal. *See notes below for additional items required to be uploaded by non-Monash applicants and Graduate Certificate in Pharmacy Practice applicants.

2. Submit your Pharmacy Practice Application Supplementary Information Form. You will require your online applicant email address or Monash student ID number to complete this form. You will be required to provide details of your preceptor, including contact email address.

 *Non Monash University graduate applicants must also upload via the online application portal and then send certified copies or original documents of:

• Academic transcripts of offical results for previous pharmacy tertiary study which indicates that you have graduated or met course requirements to graduate
• Proof of residency status such as,  your birth certificate, residency visa or citizenhip certificate
• ACER applicants are required to provide a letter from the registration board.

*Graduate Certificate in Pharmacy Practice applicants must also upload via the online application portal:

• A curriculum vitae (CV) Please include on your CV all pharmacy related work experience, other work experience, pharmacy related committee involvement and community involvement. Please do not include any student placements that are a component of the Bachelor of Pharmacy/Master of Pharmacy.
