How to apply
Applicants are required to apply online, via the Monash e-admissions portal, ensuring all supporting documentation is uploaded with application. When originals documents or certified copies are requested, they should be submited with a completed coversheet, by mail to:

Note: online applications are not available for single unit or cross institutional enrolments.
Course specific application information
Graduate Certificate in Pharmacy Practice (Internship) (3460)
Monash graduates must complete the following steps:
- Complete the online e-admissions application and upload documents:

Curriculum vitae (CV), comprehensive and current employment history, including all pharmacy related work experience, committee and community involvement and other work experience. Do not include any student placements that are a component of the Bachelor/Master of Pharmacy.

Proof of residency status (birth certificate, passport or visa).

Graduates from universities other than Monash must complete the following steps:
- Complete the online e-admissions application and upload documents:

Curriculum vitae (CV), comprehensive and current employment history, including all pharmacy related work experience, committee and community involvement and other work experience. Do not include any student placements that are a component of the Bachelor/Master of Pharmacy.

Overseas practitioner from a country other than New Zealandare required to provide a letter from the registration board.

Applicants must complete a single unit application form and provide the following documentation:

- Academic transcripts (certified copy).
- Proof of professional registration.
- Proof of residency status - certified copy of birth certificate, passport or visa.
- Proof of English language proficiency (if applicable).
- Where specified in unit outlines, you must provide evidence of meeting unit prerequisites.
